Answer
Front wing failure caused Ratzenberger's fatal crash. During qualifying for the 1994 San Marino Grand Prix, a section of his Simtek's front wing detached at high speed, causing him to lose control and crash into a concrete wall. The impact was so severe that he died instantly from a basilar skull fracture. The failure of the front wing was attributed to a design flaw and inadequate testing.
