Answer
Columbia was a Space Shuttle that was destroyed in 2003 when it broke apart upon re-entry into Earth's atmosphere. All seven crew members, including Israeli astronaut Ilan Ramon, were killed. The cause of the accident was determined to be a failure of the shuttle's left wing insulation, which allowed hot gases to penetrate the vehicle and cause it to break apart.
